Position : Full/Part Time Care & Support Worker

You will be expected to provide a personalised home care and support service to Service Users living in and around the Ilford, Redbridge, Barking and Dagenham, Havering , Newham ,Tower hamlets and Hackney areas and as a Care and Support Worker you will be expected to provide for example, personal care and hygiene, food preparation, meals, medication, shopping, housework and social support.
We genuinely welcome enquiries from those with no care experience but you must be able to demonstrate patience, empathy, reliability, dedication and a desire to improve the lives of those who are wholly dependent on our Home Care and Support Services. There is good career progression and training is provided
Position: Full/Part Time Care Coordinator(s)
These are interesting and varied roles working in a busy Home Care office that due to expansion is now open seven days per week. The roles involve on-call responsibility and weekend work on a rota basis. A full drivers licence along with good IT skills and the ability to work within a rapid moving environment meeting deadlines is essential for these positions.
The successful candidates will coordinate the delivery of a safe and well-planned domiciliary care service that is Service User focused. The positions include the management of both Service User and staff rotas using a computer based roster system, taking responsibility for ensuring that calls are fully covered as well as providing Care and Support Worker with the necessary information related to all Service User thus enabling Care and Support Worker to deliver a personalised, safe and caring service.
You will be on occasions expected to supervise Care and Support Worker and assist with Service User assessments and reviews. Similarly cover some Service User care visits.

Position: Full/Part Time Field Supervisors /assessor(s)
These are exciting times and we wish to recruit Field Supervisors to join our expanding team. You will be responsible for creating and maintaining person centred support plans, and ensuring high quality, individualised care and support is provided to all Service Users. You will also promote good practice at all times and act as effective and positive role models.
As Field Supervisors you will also be responsible for reviews, spot checks and the roles will include auditing of various service related records.
Ideally a car driver, good communicators, experienced in care with ideally an interest in training which will assist you to work alongside and support Care and Support Worker
Position: Full/Part Time ECM Officers
These are important roles within Rhythmic Care and the main objective of these positions is to monitor and reconcile all Home Care call data on an ongoing daily basis, using our in-house IT monitoring systems. Furthermore you will be expected to record, report and assist with the analysis of all data ensuring Rhythmic Care fulfils its various contractual requirements. Therefore we are looking for individuals with the aptitude to continually improve our service and implement quality control procedures.
You need to be team players, possess good communication skills, be able to prioritise and work under pressure. Equally possess excellent telephone manners and good IT skills.
Position: Full/Part Time Complex Care Co-ordinators
These roles are for experienced Care and Support Worker / Field Supervisor who have a genuine interest in complex needs and end of life care.
The roles will include co-ordination of services, assessment of Service User’s needs, formulation of personalised care plans and ensuring there is continuity of care and service.
You will be excellent communicators both verbally and in writing, be able to prioritise and work alongside other external agencies such as local hospitals, hospices, District Nurses, GPs and Social Services, as well as Service User’s family members and advocates..
You will manage and co-ordinate a team of experienced Care and Support Worker taking responsibility for dealing with performance issues, conducting supervisions and or appraisals.
